Recent incidents have shown that near misses are not always documented when shifts are busy. If reports had been submitted promptly, patterns would have been detected before medication errors increased in Ward C.

Effective immediately, all staff are requested to complete the electronic form within two hours of an event. Identifiers will be anonymised, summaries will be reviewed weekly, and action plans will be assigned to unit managers. Trainers were told to demonstrate the workflow during induction sessions.

Supervisors asked whether barcode scanners had been maintained after the budget freeze. Engineering confirmed that devices were serviced, though spare units had not been ordered since March. Procurement was instructed to restore stock levels and not to delay orders when safety equipment is involved.
