The overnight shift in thoracic surgery teaches grammar unconsciously because crises reorder speech before they reorder priorities. Rarely have I witnessed such precision under pressure as I did when the on-call team received two critical admissions within eleven minutes.

Never before had the ward sounded so quiet after the pagers stopped. Only when the second patient was stabilised did anyone mention coffee. Not until the registrar confirmed imaging did the consultant relax her shoulders.

What the first patient needed was immediate airway assessment. What the second patient needed was reassurance that staff were not abandoning her while colleagues worked behind curtains. All the charge nurse wanted was clear handover language, and all handover language must front life-threatening facts first.

It was the anaesthetist who spoke with calm authority. It was her checklist that prevented a medication error someone else might have made while rushing.
