Reception note: Patient Leo Marin, age thirty-four, appointment at 11:20 for cough and fatigue.
Dr Walsh: Good morning, Leo. What brings you in today?
Leo: I have felt tired since Monday. I have also had a dry cough, especially at night.
Dr Walsh: Have you had a fever at any point?
Leo: I have not measured high temperature, but I felt warm on Tuesday evening.
Dr Walsh: Did the cough start suddenly on Monday?
Leo: It started slowly on Sunday night. Monday morning it grew worse. Yesterday I coughed during a work call, and my manager noticed.
Dr Walsh: Have you ever had asthma?
Leo: I had mild asthma as a child. I used an inhaler until I was twelve. I have not needed one since my teens.
