Diary Entries During a Hospital Internship

November 3. My clinical rotation in the acute care ward of St Jude's Teaching Hospital commenced at six this morning under a cold autumnal drizzle. The transition from medical textbooks and laboratory simulations to emergency clinical diagnosis is exhilarating and terrifying in equal measure. Senior consultant physicians move through the crowded wards with calm authority; meanwhile, novice interns like me double-check drug dosage calculations and patient wristbands with intense concentration.

In addition to performing routine morning blood draws and charting vital signs, clinical interns must learn to interpret subtle non-verbal signals from anxious family members. For instance, an elderly patient admitted this morning repeatedly denied having chest discomfort, yet her shallow breathing and tense posture told an entirely different clinical story. Consequently, our supervising registrar ordered an immediate bedside electrocardiogram that revealed an acute cardiac event.
